DOT/SAP Assessments

 I am a federally-recognized Substance Abuse Professional (SAP), qualified to perform assessments on Department of Transportation-covered employees who have failed a drug or alcohol test.

 If you are an employee covered by DOT regulations and have failed a drug or alcohol test, it is required that you be assessed by a SAP, and complete treatment recommendations before you can return to safety-sensitive duty.

The Process

Schedule an appointment with me for your initial evaluation. At the end of this session, I will refer you out to treatment resources near your home. I will notify your employer that you have been assessed.

After you complete the recommended treatment, you will return to me for your follow-up evaluation. At the end of that session, I will determine whether you have successfully completed all requirements. If so, I will then issue a letter to your employer stating that you are released to return to safety-sensitive duty. I will also issue to your employer guidelines for follow-up testing that must be performed over the next two years.

What to Expect

Please come into my waiting room and complete the paperwork that will be waiting for you there. Payment by cash or money order only will be required at the start of our session. Insurance does not cover SAP evaluations.  At the end of our session, I will give you the name and phone number of the treament center or counselor who you will see to complete your treatment. Once you have completed the treatment, you will contact me to schedule our second and final appointment. At the conclusion of that session, I will send notification to your employer that you have completed treatment and are ready to return to safety-sensitive duty.
